Joinder – Directions for Submitting

1. Print the Joinder, fill in the appropriate information, then sign it. Send the completed Joinder, along with your check, to Four Mile Church by *Sunday, November 10th, 2024. Please print legibly, making sure all information is accurate.

NOTE: If you are using the tax liability for you and your spouse, have BOTH sign and list BOTH Social Security numbers.

* The “allotted time” is that date by which you need to return the Joinder and check to the person who provided the Joinder to you. A late submission may result in rejection.
